To register by mail, please download the documents below to your computer.  They are available in either  'Word' or 'PDF' file formats. 

     Read the Application Procedures document carefully.  Be sure to note the information regarding USAH member number needed for your application.  Also, a special note to goaltenders-  Please contact Coach Nick Fatis prior to submitting your application.  He will advise your regarding availability of goalie slots for the tryout you are interested in attending.

     After reading the procedures, complete the application thoroughly, include your check  or money order (credit cards not accepted by mail) and be sure it is postmarked prior to the deadline for discounted early submissions.  You may wish to contact Coach Fatis to confirm that your application has been received prior to the date of the tryout.
